as Jon asked, it may help to know how the example was compiled.
On the other hand it seems there is no difference if the example is compiled using bmake from Develpment Shell or directly from Visual Studio when sln file is opened.
Can you check ManagedToolsExample.dll exists in MicroStation\Mdlapps folder? If you use Visual Studio, it's not copied to the folder automatically.
